Office Policies
Payments: I prefer cash but also accept all major debit/credit cards for treatment by new patients. I also accept personal checks from returning patients.
Health Insurance: To provide you with the best service possible, I do not directly bill insurance companies but will provide a superbill for your insurance company upon request. If you are insured, I recommend contacting your health insurance company to see if they cover services by an out of network naturopathic physician. If they do, you may submit your superbill to your insurance company for reimbursement.
Cancellation Policy: If you must cancel or reschedule, please do so by phone or email at least 24 hours prior to your scheduled appointment. If you neglect to advise me that you are unable to make your appointment, I require a credit card number to reschedule. If you miss the second appointment, I will charge a $50 missed appointment fee to the card you provide. I reserve the right to make exceptions to this rule if there are extenuating circumstances.
Fees
Note that there is a 10% discount for active duty and veterans of the US armed forces. You must let us know that you qualify for this discount when you book your appointment.
NCR fees
NCR-New Patient: $700 4 day series. This visit includes new patient evaluation. If you are already my patient and we have discussed your condition, you can begin NCR at the return patient rates.
Return Patient: $600 4 day series
Disabled/Elderly (fixed income) with confirmation of fixed income or disability: $600 new patient 4 day series, $500 return 4 day series.
Children receive a discounted price depending upon age. They are 1/2 price up to age 3 and 2/3 price from age 3 to 10. After age 11, they are the same as adult prices.
Individual Consultations
Medical Marijuana Renewal/Evaluation only $100, Distance appointment $135 (with at least 2-3 medical records from your primary physician documenting care and diagnosis). Please note that this appointment is NOT reimbursable by insurance.
New Patient Specialist visit- medical marijuana focus. $200, Distance Appointment $235
New Patient Whole-Person Naturopathic appointment $300, Distance Appointment $335
Return visits in-person, Skype or over the phone are billed to the nearest 10 minute increment. $50 for 10 minutes, $100 for 20 minutes, $150 for 30 minutes, $200 for 45 minutes, $250 for one hour.
Miscellaneous Fees
Records request fee $25 if we need to obtain records from your doctor’s office. There is NO FEE if you supply your own records. Records may be faxed or emailed in advance, or copies may be hand-carried to your appointment. We are happy to supply standard mail, fax or e-mail copies of your records upon request for a $25 fee.
Mailing fees- standard mail with delivery confirmation of medical marijuana recommendations are included in your visit fees. In WA state, it typically takes 1-3 days for standard mail to arrive. Overnight delivery is available for an additional $20.
